TERRACE BAY, ON – Nosh/North Shore Family Health Team Update.
Shots provided to:
- Everyone associated with LTC and Chronic Care
- the 80+ population
- other health care workers
If you are in any of the above priorities have submitted your consent and have not received a shot or an appointment for a shot on April 1st, please call 823-3828 and leave a message, someone will get back to you within 24 hrs.
If you are in any of the above groups and have not dropped off a consent, please complete one and submit it.
Consents are available on the North Shore Family Health Team website (CLICK HERE to download NSFHT consent form) or can be picked up at J.E. Stokes medical center in Schreiber or the McCausland Hospital in Terrace Bay. They can be submitted to the same locations.
SCHEDULED VACCINATION CLINICS
April 1
- remainder health care workers
- Indigenous 55+
- Chronic Home Care recipients
- Adults 75-79
If you are these priorities and submitted a consent before March 23 and have not received a call, please contact 823-3828. If you are in the above priorities and submitted a consent between March 23 and March 28th, we hope to call you this week. If you fall under these priorities and submitted a consent after March 28, you may have to wait until the next vaccine clinic for an appointment.
April 8 (Tentative)
- Adults 70-74
We have had many questions regarding the following groups:
- Indigenous under 54 (we are waiting guidance from TBDHU for when you will receive your vaccine)
- 60–69-year-old
- People with chronic health conditions
- Essential service workers
You are all listed as priorities in the Ministry of Health’s Phase 2 document, but we have not been given any guidance locally regarding the order that these groups will be vaccinated in. Within these groups there are also categories. You can search the Government of Ontario Phase 2 vaccine roll-out plan for more information.
Please submit a consent with your group listed on the top to help us schedule once directed to do so, and please fill in your age beside your birthdate.
