COVID-19 Vaccination Clinics Update: MANITOUWADGE

MANITOUWADGE, ON – Santé Manitouwadge Health (SMH) has been using our Health Care Record data base to reach out and offer the vaccine to all our residents in order of priority as established by the Ministry of Health and in coordination with the Thunder Bay District Public Health Unit.
If we have contacted you and you have already completed and returned a consent form to us, you do not have to do anything else. You will be contacted when it is your turn.
Priority Groups Eligible For Next Vaccine Clinic
- Those 80 years of age and older
- Adult recipients of chronic home health care (as determined by the LHIN)
- Indigenous individuals 55 years of age and older
If you fall in the priority group above and we have not yet called you, please complete the consent form linked below and return to SMH FHT/clinic office now to be included in our next clinic expected within weeks. Completed consents can also be e-mailed to covid19info@mh.on.ca .
For any other resident who wishes to receive the vaccine, feel free to complete the form (click here to download the English consent form) and return it to us. Once received, we will register you and when the vaccine for your group is scheduled, we will call you for an appointment. Clinics are scheduled as the vaccine becomes available. Do not delay returning your form as we may not have enough notice to process new forms for the next clinic.
