RE: Manitouwadge Diabetes Health & Wellness Program
As of December 1, 2012, all clients who received services from the Northern Diabetes Health Network (Manitouwadge Diabetes Health & Wellness Program) will now be provided those services from NorWest Community Health Centres. NorWest Community Health Centres has a long and successful record of providing care to communities in the District of Thunder Bay. We are familiar with the services of NDHN and have partnered with them at our Longlac site and in the district. Please check our website for additional information www.norwestchc.org.
We are pleased to announce that staffing will remain the same and all services will continue in the community of Manitouwadge as they did under the former NDHN. All client files will be transferred to Norwest Community Health Centres to ensure continuity of care, security and privacy for all clients.
